Camby Auto Repair Service Contract
This Service Contract (“Contract”) is entered into on [Date] between:
Camby Auto Repair, located at [6825 S Raceway Rd, Camby, IN 46113], hereinafter referred to as “Repair Shop,” and
[Customer Full Name], residing at [Customer Address], hereinafter referred to as “Customer.”
WHEREAS, the Customer owns a vehicle requiring repair services, and the Repair Shop agrees to provide such services under the terms outlined below.
NOW, THEREFORE, the parties agree as follows:
1. Vehicle Information

2. Scope of Services
The Repair Shop agrees to perform the following repair services on the Customer’s vehicle (“Services”):

3. Payment for Parts
• The Customer agrees to pay the full cost of all parts and materials required for the Services upfront, prior to the commencement of any repair work.
• The estimated cost of parts is $[Amount], as detailed in the attached estimate (Exhibit A). Any additional parts required will be communicated to the Customer, and payment must be made before installation.
• Payment for parts shall be made via [accepted payment methods, e.g., cash, credit card, check, zelle, cash-app].
• Pursuant to Indiana Code § 9-22-6-2, the Repair Shop will provide a written estimate for parts and obtain Customer authorization before proceeding with repairs.
4. Labor Costs and Financing
• The Repair Shop will provide all labor necessary to complete the Services.
• The estimated labor cost is $[Amount], as detailed in the attached estimate (Exhibit A).
• The labor cost will be financed by the Repair Shop, and the Customer agrees to repay the labor cost in installments according to one of the following payment schedules, to be selected by the Customer:
• Weekly: $[Amount] per week for [Number] weeks.
• Biweekly: $[Amount] per two weeks for [Number] biweeks.
• Monthly: $[Amount] per month for [Number] months.
• The Customer must select a payment schedule at the time of signing this Contract by initialing the chosen option:

• Payments shall begin on [Start Date] and continue until the labor cost is paid in full.
• Payments shall be made via [accepted payment methods, e.g., cash, credit card, zelle, cash-app, bank transfer].
• Late payments may incur a late fee of $[5] per a day, not to exceed the maximum allowed under Indiana law (Indiana Code § 24-4.5-2-202 for consumer credit transactions).
5. Storage Fees
• If the Customer fails to retrieve the vehicle within [3] business days after completion of the Services and notification by the Repair Shop, the Customer agrees to pay a storage fee of $10 per day, as permitted under Indiana Code § 9-22-6-3.
• Storage fees must be paid in full before the vehicle is released to the Customer.
6. Mechanic’s Lien Rights
• Pursuant to Indiana Code § 9-22-6-1 et seq., the Repair Shop may retain possession of the vehicle until all amounts owed for parts, labor, and storage fees are paid in full.
• If the Customer fails to pay any outstanding balance within [30] days of written notice, the Repair Shop reserves the right to exercise its mechanic’s lien rights, including the sale of the vehicle to recover unpaid amounts, in accordance with Indiana Code § 9-22-6-4. The Customer will be notified in writing of any intent to enforce a lien, as required by law.
7. Additional Terms
• Authorization: The Customer authorizes the Repair Shop to perform the Services and to order necessary parts on their behalf.
• Warranty: The Repair Shop provides a warranty on labor for parts as per the manufacturer’s warranty, if applicable. Any issues covered under warranty will be addressed at no additional cost to the Customer, pursuant to Indiana Code § 24-5-13 (Motor Vehicle Repair Act).
• Changes to Scope: Any changes to the scope of Services or additional repairs will require Customer approval in writing and may result in additional parts costs (to be paid upfront) or labor costs (to be financed per the terms above), as required by Indiana Code § 9-22-6-2.
• Default: If the Customer fails to make payments as agreed, the Repair Shop may suspend work or retain possession of the vehicle until payments are current, subject to Indiana law.
• Termination: Either party may terminate this Contract with written notice if the other party breaches its terms. Upon termination, the Customer remains liable for any outstanding payments for parts, completed labor, or storage fees.
• Consumer Rights: The Customer is entitled to rights under the Indiana Deceptive Consumer Sales Act (Indiana Code § 24-5-0.5). Any disputes regarding deceptive practices may be reported to the Indiana Attorney General’s Office.
8. Liability and Risk
• The Repair Shop is not responsible for any loss or damage to the vehicle caused by factors beyond its control, including but not limited to theft, fire, or natural disasters, while the vehicle is in its possession.
• The Customer acknowledges that the vehicle may not be returned until all upfront parts costs, outstanding labor payments, and storage fees are paid in full.
9. Governing Law
This Contract shall be governed by the laws of the State of Indiana, and any disputes arising hereunder shall be resolved in the courts of Marion County, Indiana.
10. Entire Agreement
This Contract constitutes the entire agreement between the parties and supersedes any prior agreements or understandings, whether written or oral.
11. Signatures
By signing below, the parties acknowledge that they have read, understood, and agree to be bound by the terms of this Contract.
